SEOUL — South Korea’s total coronavirus infections topped 10 million, or nearly 20% of its population, authorities said on Wednesday, as surging severe cases and deaths increasingly put a strain on crematories and funeral homes nationwide.
The country’s infection and death rates are still far below those recorded elsewhere, as almost 87% of its 52 million residents are fully vaccinated and 63% have received booster shots.
But the death toll nearly doubled in just about six weeks, with daily fatalities peaking at 429 last Friday, fuelling demand for funeral arrangements.
The government on Monday instructed the 60 crematories nationwide to operate for longer hours to burn up to seven bodies from five, and the 1,136 funeral parlors capable of storing some 8,700 bodies to expand their facilities
Why are Philippine case rates so low when everyone else in the world is spiking?
It is likely multifactorial, and is a product of both science and luck. These include:
1. People following mask mandates.
Pinoys in general have worn their masks faithfully, and there is wide support for retaining mask mandates. Face shields likely helped, especially when vaccination was just starting out. Some continue to wear them. Even now people still feel naked without their mask when eating out, and I see people putting it right back when they go to get food in buffets. This gives us an extra layer of safety, which is always a good idea.
2. Many vaccine types.
Most countries only use two to three vaccine brands. The US for instance only has 2 mRNA (Pfizer, Moderna) and one viral vector (Janssen). With the rise of variants, too much emphasis on spike protein increases the risk of antigenic escape. We use eight different brands. Despite the Western bias against Chinese brands, the inactivated vaccines (Sinovac, Sinopharm) actually have more antigen epitopes aside from Spike protein. This makes them more likely to be resilient to variant mutations.
3. Early boosting.
The Philippines was one of the earliest countries to adapt boosters at 3 months. The TAG and All Experts Group very quickly reviewed Omicron data and adopted the recommendation, just in time for the Omicron spike. In addition, because of our vaccine brand diversity, many people ended up with heterologous boosters which may be more effective against variants than homologous ones.
4. Our Omicron spike was BA.2.
Unlike most countries in the world that got BA.1, our January spike was BA.2 driven. We saw mostly BA.1 among quarantined travelers, which means they didn't make it to the community. Unfortunately someone with BA.2 made it through. This resulted in a large spike but it also quickly receded. It probably ran smack into a boosted or recently vaccinated population. We also delayed our Delta spike with preemptive lockdowns so the hybrid immunity from breakthroughs from August was still relatively fresh. We expect BA.2 hybrid immunity (BA.2.3 to be exact, since BA.2 in the Philippines has been designanted its own sublineage) to protect against other BA.2 sublineages, including BA.2.2 which is wreaking havoc in Hong Kong.
There is no way to figure out which of these had the greatest impact, but what is very clear is cases are staying low despite opening up and our hospitals are not overwhelmed. There may be other factors but these four are the ones that make the most sense. Let us not become complacent though as a spike is always possible. If that does happen, we have protocols to respond to it. In the meantime, enjoy your new-found freedom, but don't take it for granted. Keep masking up and vaccinating and we will be ok.

Could it be the booster shot? When I was in the US a couple of weeks ago, we were no longer wearing masks and no spacing (hotel, office, mall, streets, restos, eveywhere!) except when you are inside an airport. We even shook hands and sometimes hug with acquaintances at times. We were required to undergo rt-pcr and 1 even did a repeat before the flight as his specimen was taken beyond the 48 hour window. We were all negative and even now when I checked on them, are doing fine (except for the jet lag). We were all boosted with mrna before the trip.
