As a safe rule, diesel fuel should be disposed of if stored for more than 8 months. Try this: put fresh diesel fuel in a small container, connect your fuel suction and return lines to this container, and run your engine. If performance improves, then you know that your problem is due to a fuel quality issue.
to add up some info. with the truck not running (engine on), blipping the throttle produces a combination of severely white and not so severe black smoke with the engine running roughly. Check pump timing, valve clearances, and air intake system for restriction. You can also bring your van to a qualified diesel workshop.
