Car jerks when shifting from Park to Drive or to Reverse

What could be the problem of an A/T Tranny if the car jerks a little when shifting from park to drive or to reverse etc? (Whenever the tranny is shifted)

Gaano po ba kalakas ang jerk because it's normal for an A/T vehicle to jerk a little when shifting from P to R or D and vice versa.

Aside from the above advices given I suggest you to take a look at your engine and transmission mounts just to make sure.
