Car jerks when shifting from Park to Drive or to Reverse

What could be the problem of an A/T Tranny if the car jerks a little when shifting from park to drive or to reverse etc? (Whenever the tranny is shifted)

aside from the suggestions stated

have you tried pressing the brake pedal a bit deeper (to make sure you are in full stop) before shifting from park to drive or to reverse?

since P and R gears requires you to press the brakes before engaging them.
