From a stop, I step on the accelerator with moderate pressure to speed up the shifting until the highest gear (4th gear in my case), but not too much pressure - just enough to get it to shift at 2000-2500 rpm.
Once I'm in my cruising speed of around 80 kph, I put as little pressure as possible on the accelerator. Think of it as squeezing a shampoo bottle as lightly as you can so that you only use a minimal amount of shampoo. I find that this is better than stepping on the gas hard, then coasting, then stepping hard again.
On uphills, I try to maintain the same minimal pressure, but there are times that the speed drops to 60 kph which isn't very efficient anymore, so I apply a bit more pressure even if it means I'll have to rev a bit more. This is still more efficient than crawling at a low speed. On downhills, I still apply the same minimal pedal pressure as on a flat road, which means the car goes faster than 80 kph. I use that momentum so that I carry speed when I traverse the next uphill.
When approaching toll booths, I coast from as far as 500-750m away so that by the time I reach the queue, I'm only doing 40-60 kph so I minimized my braking and hence my fuel use.
Just a reminder though, it's a really, really boring drive to stay at 80 kph even if the road is completely empty. Everyone else will just overtake you. I find this kind of driving impractical since it makes my 2 hour trip a 3 hour trip, all for a few pesos of savings. I just do it to see how efficient my car can go, but it definitely isn't my kind of driving.
