Taal's Alert 2 was raised just last Sunday.

But the urgency was there.

No one saw that coming.

These things can escalate very quickly like Taal now or can go slow like Mayon. Like an illness. Sometimes it can overtake the human mind as well as any equipment.

Taal has been monitored since the 1950s. Pagasa and Phivolcs are in a situation where if it issues a warning and nothing happens, people will deride them. And some people even find them an annoyance. E.g. hazard maps and stupid building codes are in the way of a sale and making profit.
