Blow by means crankcase gasses are escaping into the CCV, most cars have very little amount of blow by even from factory, that's why there's a CCV (crankcase ventilation) system usually routed back into the intake manifold for burning.
But when the amount of blow by is severe (as in puffs of smoke or severe loss of oil within a short amount of time/distance traveled), then it is considered for engine overhaul or rebuild.
