You should attached the clamps first before starting the car. If you're using cigarette lighter port, this may not have enough power to start the car, worse you may overload it.

if one clamps on the clamps onto an already-running engine,
a strong spark might startle you into doing something that you shouldn't do.
On jump cables, heres what I'd do:
1. + discharged to + support
2. - support to - discharged or body ground to body ground.
This ensures that the battery sensor is not bridged.
3. Start & fast idle support vehicle
4. Start discharged car
5. Before disconnecting jumpers, turn on lights, rear defroster, highest A/C for 10sec to prevent voltage surge
6. Remove in this order:support ground, discharge ground, support +, discharged +

my car's battery died,
because i left the ignition on for a few days.
i recharged it using my small basic car battery charger, so's i can start it and drive to the battery store. the charger's meter initially said i was putting in only 4 amperes.
i feared the worst, as it's over three years old.
but it started, after some hesitation.
the store guy there tested the battery. "ok pa sir. i-skyway nyo lang."

What i meant are those 12 V sockets that can be directly clamped to the battery. I bought an Elevo tire Air Pump but it blew the fuse of our Forty's rear 12 V socket, don't wanna blow off the other sockets so I bought a 12 V socket that can be clamped directly to the battery terminals.

that happened to me, too.
with the ignition at ACC, i plugged my elevo tire inflator into my innova's cigarette lighter socket.
it also blew my fuse.
i have no explanation.
since then, i insert the plug into it, before i turn the key. but only for the inflator. for my electronic gadgets (cellphone charger, etc), i still plug and unplug with wanton abandon, with the car running.
and in case the fuse blows again, "minarkahan ko na yung fuse, so's i may replace it easily."

I imagine it's an alligator clip to female cig lighter socket, like this...
That is used to bypass the accessory cig lighter sockets because, as many of you found out, it will likely blow the fuses to kingdom come. The above cable is used so that the accessory can be attached and powered directly from the battery.
On whether you need the engine running or not depends on how powerful the accessory is and/or how long you plan to use it. Attach the alligator clips with the engine running or not DOES NOT matter. Just do it safely (ie. correct polarity, cables does not snag any moving parts or the alligator clips accidentally detaching and gets sucked in the accessory belt... these sort of common sense).
