I did. Walang lock. What happened is I wanted to check the engine braking sa downhill so I switched from D -> M1 then pagdating ko sa baba, I switched up all the way to reverse instead of right. When I stepped on the gas, na-off ang engine.

Yout gate shifters look like this. So from M+, to reverse, you need to do the following:

Go to D
Move the shifter up to neutral,
Move the shifter left then move to shifter up to reverse.

All these while stepping on the brake. Mahirap na accident ito imho....

After I reached the flat, the car was at crawl speed. Then I mindlessly set the tranny to R. No brakes were pressed. But in theory, if this happened only once, may damage ba sa tranny?

Sometimes, speed humps auto changes my tranny from D to N. Happened twice already.

Physically nag momove ang shifter from d to n?
Report mo na sa casa right away. Video mo while going to speed humps to see and show it to casa.

if i have to guess,
"your transmission is malfunctioning."
bring it to the dealer before they charge you fixing fee.

Yes. Pag dating ko sa hump, I slowed down just enough para maka climb si front wheel. Once cleared, I lightly tap the gas para may enough speed si rear wheel to climb as well. It’s as this point na biglang tumaas ang sound ng makina. Naka N na pala. I’ll save the video next time when it happens again. Thanks.
