I used to process applications (though not passports obviously hehe) but I think it is very similar. We process kasi based on the queue of applications. So application 1 ang mauuna sa lahat. Kung gusto mo ng expedite you will jump the queue hindi naman lahat pwedeng expedited. Though dapat humahanap pa rin sila ng paraan to improve the processing time. Unless optimal na yung current (which is unlikely)

Depends, the only way for that to happen is if they purposely threw it away. Lets just say when my windows are open and some bills land inside my car i wont break speed limits just to return it if they dont stop.

AFAIK sa US you can get passport from multiple departments of government. Hindi lang DFA, pati yata sa postal office makakakuha ka ng passport.

Pero sa tin parang di pwede yun. Sa daming illegal passport sa tin.
