    If I feel that it's an honest mistake, I call over the manager, and point it to their attention. Usually, the food is replaced immediately, and more often than not, also removed from the bill. If there is no remedial action taken, I will ask that the item be removed from the bill, pay the remaining items, then leave immediately - after making a big fuss, of course - and never come back to that restaurant.
