    IMO, in both cases it is pointless not to have both the printed name and signature of the seller in the DOS (since these two verify the personality of the seller/orig owner). Laging dapat meron yang dalawang yan. Try mo ipa-notaryo na walang printed name i.e. signature lang or wala both, malamang hindi yan i-process ng notary public.

    thanks. how about for point #1, the DOS contains the signature of the seller but no handwritten name, address etc. And does having copies of the original owner's IDs (drivers license and TIN ID with 3 specimen signatures) gives any support kahit walang handwritten name sa DOS?
