affidavit of loss tapos punta ka sa LTO office na malapit sa iyo.
meron nang LTO sa mga malls like galeria and sm-tiendesitas.
parang renewal din yata ang charge dyan?

Execute an affidavit of loss, bring any ID with a picture in it then photocopy both documents and that's it wala ng iba pang kelangan, and go to the nearest LTO Office and fill-up the green form there for renewal/lost licensed. The cost for replacement is P277.25. They call it duplicate processing. The waiting time for the replacement is 1-2 hrs. Beware ka lang sa mga fixers at mga gago na nakaistambay doon. Alam nila kung sino ang bibiktimahin, deal only with the legitimate LTO personnel para di ka maloko at mabiktima.

I suggest going to the same LTO Office where you had applied for your license ... they need to search their records of the last license that was issued to you.

they can easily issue you a replacement paper license but depending on the LTO branch, you may need to wait for a few months before they can release the actual credit-card type license.

Noong araw, pag di mo alam yung number ng license mo, hahanapin pa yung record mo among the room full of folders ng LTO. Permenent daw kasi yung license number eh. Guess who does the searching? Ngayon ba computerized na? Hindi na kailangan yung number? Up to now kasi xine-xerox ko pa yung license ko then file sa office ko eh.

My license got lost recently and I go to the Main Office nila dun sa East Ave., madali lang naman kumuha ng duplicate ID kya lang yung affidavit of loss cost P100.00 sa mga fixers na nakatambay dun wherein its only P50.00 dun sa mga normal at legal na transakyon talaga. Tapos photocopying of any ID with picture costs you P2.00. Di naman nila kinuha yung original na affidavit of loss kundi yung photocopy lang at photocopied ID lang. Tapos papuntahin ka nila dun sa SMAIG para kunin yung print out ng file number at Licensed NO. mo. All in jiffy yun, yun nga lang masungit yung guard dun sa computer center nila na kung umasta e sya ang amo mo. Then you return it sa kumukuha ng picture para sa ID at tatawagin ka sa counter 6 kung saan magbabayad ka ng P277.50 at maghintay ka na lang na marelease yung card mo at yung official receipt sa counter 5. It takes only 1 hour for me to do all these kaya no need na pumunta ka sa isang fixer na who will do these things for you. Tyagaan lang yan dre.

I lost my Philippine License and I want to get one or replacement, how can I do that? Problema is nand2 ako sa US and after 2yrs pa uwi and kailangan d2 ung phils license ko para maka drive

OT lang guys. Ang mahal na pala ngayon kumoha ng license. from studnt license to non pro i think more or less P2000 na ma gastos mo. to start off P317.63 yata and studnt license. then pag non pro P417.63.. thats what is officially on the LTO website.

kaya lang it doesnt include the drug test and medical test.. plus at LTO tagaytay mai pratical driving test na, that cost 400 for car and 400 din yata for motorcycle.. im not to sure bout the details but my frend told me ang gulo daw talaga mag kuha non-pro sa tagaytay LTO..

Reviving this thread. I lost my license with its OR. I have a xerox copy of it kaso yun yung bago pa ako mag renew last year. Yung may xerox copy ako yung 2012 expiry license ko. Okay lang ba yun? Lalagay ko lang sa Affidavit of Loss ko yung license number na nakalagay dun sa xinerox ko na lumang license ko. Iisa lang ba license number natin kahit mag renew pa? Thanks, sorry daming tanong.

Sir punta ka lang sa kahit saan lto dala mo yung affidavit of loss mo, ma trace na nila sa system yung license mo then i reprint lang nila yun wala pa 1 hr. makuha mo na rin.

Yung license na card na ba mismo makukuha ko sir? Sabi kasi nung iba parang certificate or basta papel lang muna, di raw agad makukuha lisensya na card. Thanks po.

I lost my license too. Then nagpagawa ako ng duplicate sa LTO. Nakuha ko rin naman yung license card.
