yes. prepare an affidavit of loss. i really dont know kung sa east avenue kukunin. but afaik, pwede kung saan ka huling nagparenew ata.

Always make a habit to photocopy your cards... mas mabils processing pag may attach na photocopy.

lost my license recently, you need an affidavit of loss, prepare to remember when & why nawala licence mo, year issued. you also need cedula for the affidavit.

i went to main branch east avenue that time and spent half day just to tell me that i can only get a replacement card from the lto branch where my first license was issued. prepare at least 1 working day to settle this.

dun sa last na pinagrenew-han mo sya ia-apply...kelangan mo nang affidavit of loss at cedula....tapos pipila ka lang ulit, then bibigyan ka ulit nang resibo at babalikan mo yung card after 60 days.....

ganon yung ginawa ko nung manakawan kami....doon na rin ako kumuha nang affidavit of loss at cedula sa LTO office, me makukuha ka pang tip sa kanila kung sino ang lalapitan sa loob para medyo mabilis ang processing....yun eh kung medyo mabobola mo yung tao....hehhehehe

di ba supposed to be mabilis na ngayon ang processing sa lto kasi computerized na sila?

license renewal now only takes 1-1.5 hrs. issued na sa yo ang card mo kaagad. i see no reason why card replacement would be different assuming kumpleto ka sa requirements.
