Although uncomplicated influenza-like illness (fever, cough or sore throat) has been reported in many cases, mild respiratory illness (nasal congestion, rhinorrhea) without fever and occasional severe disease also has been reported.
Other symptoms reported with swine influenza A virus infection include vomiting, diarrhea, myalgia, headache, chills, fatigue, and dyspnea.
Conjunctivitis is rare, but has been reported. Severe disease (pneumonia, respiratory failure) and fatal outcomes have been reported with swine influenza A virus infection.
The potential for exacerbation of underlying chronic medical conditions or invasive bacterial infection with swine influenza A virus infection should be considered.

Symptoms of swine flu are similar to those caused by other influenza viruses. These include fever and chills, coughing, sore throat, and fatigue. Sometimes nausea, vomiting, and diarrhea can occur.
In the past, pneumonia and potentially fatal respiratory failure have been associated with human swine flu infections. It is not possible to distinguish swine flu from "regular" influenza on the basis of symptoms alone.
The symptoms of swine flu can also vary in severity among affected individuals.

from what i've read, those who died of H1N1 infection usually had underlying medical conditions (like heart disease)
those people died of complications
high fever is bad for people with heart disease
