So kung by design yung SCV ay nakakabit sa suction ng HP pump as a dumping valve just like in Bosch design (to dump fuel back to the fuel tank) which is normally open when de-energized, that's the safe position. If won't increase on RPM when SCV stuck * open position and will lose rail pressure.
I understood your point, SCV can also be installed in series with the suction of the pump (not as a dumping valve). When OPEN is more flow, more pressure. So if this is the case, the SCV being NO is not the in the failsafe position. If solenoid fails or maybe valve stuck up mechanically , there's a tendency of high pressure. But of course there is a pressure safety valve to avoid over pressure.:-)
Are you really talking about Piping & Instrument Diagram? Or Process & Instrument Diagram?

In my years of working with our P&IDs (Piping & Instrument Diagrams) and supplier/vendors P&IDs I never seen your FC/FO/FL. We only reflect kung Energized or De-energized ang position ng valve same with our Supplier/Vendors kaya nagkakaintindihan kami.

For us kasi, we uses passive devices to keep the system safe in case of failure.

Back to the commonrail system, I think both Bosch and Denso uses a passive device called pressure limiter as the failsafe. Find it in the (schematic) diagram. Nasa dulo yan ng rail.

Again, the SCV of Strada's denso system has no failsafe sunction. When the SCV fails (stucked/loose wiring/not energized) it is on the normal (OPEN) position.
So sign of Strada SCV failure is an increase of engine power due to abnormal high pressure. Followed by power loss when the failsafe device called pressure limiter cracks open (Pressure drops from 220 MPa to 50 MPa). Power returns when the rail pressure rises back to the 150MPa to 180MPa operating pressure.
Possible na paminsan-minsan lang na mangyari ito (halimbawa natinga ang valve heheh) or some loose wirings. Pero kung sira talaga ang SCV it will be annoying to drive the vehicle pero it is not prone to stalling.

I had my strada SCV replaced at 30K mileage due to stalling.
Now (55K mileage) problem starts again.
Sometimes when I rev the engine, rpm is not responding. stucked to idle speed.
Could this be a SCV prob again?
what I do is stop the engine for a while, shutdown, then start again.
    Need help badly

Meron akong monterp GLS SE 2009 Ed. 80k mileage. If I'm not mistaken 80,259k yung mileage nung pinalitan, current is 80,412k. I'm still observing kung babalik pa yung sakit. So tama sila hindi EGR sakit, suction daw, kaya nilinisan. Kapag daw bumalik yung sakit, SCV nga talaga..

(Engine: 2KD-FTV (Hilux MK6 D4D))

Was experiencing 'sinok'. loss of power in low gears/RPM, minsan stalling. Odo is near 100K kms. Had it electronically diagnosed and result was negative. Pushed my luck and did a DIY with my SCV and EGR since expensive yung replacement parts. Its been a month since i cleaned the SCV and no signs of those symptoms yet. SCV is very easy to dismantle, cleaner costs around 200 php plus the o-rings. And don't forget to add 2T oil in your tank to keep your SCV and other fuel line components clean and well lubricated.

Thanks and have a great metro-manila traffic!

every fuel filter change, make sure you also clean the scv
