They're real. They were talking about them on TV last night.

I'm an experienced photochopper (can't say "expert", don't think I'm that good)... and it's nearly impossible to get lighting and tone to match up that exactly for half a dozen chops unless you have access to Mr. dela Hoya himself or have pictures with his head in those exact same positions, with the exact same lighting... and even then it'll be obvious... I mean, look at FHM or Maxim... those guys do it all the time, and even without adding an extra head or two, it's obvious where they've chopped, because skin tone is very hard to replicate, even on the same person.
