As for spare keys, usually its just one or two keys... we don't keep the entire keychain full of keys in the wallet.

Condom, just one when going on a date. Best not to keep it there for too long though because a "ring" would start to form on the outside of the wallet.

Medicine, I would bring just one tablet for my anti-allergy medicine. This is especially on trips abroad when i don't have quick access to medical help.

It also helps NOT to have a full photo album inside one's wallet to keep it slim.

I downsized my wallet already. My Mom gifted me a wallet to 2 yrs ago and I only bring my most used cards

    Cash
    2 BPI ATM
    HSBC ATM
BPI Credit Card (my primary credit card)
BPI Petron CC (but I ONLY gas as Shell )
BPI E Credit Card (for my online purchases)
HSBC Credit Card (purchases charge to family)
    AMEX Credit Card
    S&R Card
    M&S Reward Card
    Marketplace FSP
    Rustan's FSP
    Rustan's SSI discount card
SM Advantage Card (I did not want this because I hate SM but my Mom got if for me anyway. I get points when I gas at Shell)
    SSS ID
    Mercury Suki Card
    Intellicare Card
    Office ID

I had to get rid of all my other discount and membership cards. i just keep it in a separate card holder because that is all that my wallet could fit (16 slots)

On the slit of my wallet I have

    Mother Mary Novena
    St Josemaria Escriva Prayer
My Maternal Lola's prayer card (given on her wake)
    Dad's Business Card
    Starbucks GC
    2 Globe P100
    4 PLDT Budget Card P100
    Torch GC
    50 yr old friend's Dad's business card (need to remove this}

I don't keep receipts in my wallet because I read it is malas to keep it in the wallet. I have a separate pouch for receipts
