help: lost TIN# form

what to do when your bir form 1902 (the 1st form you fill up when you get your tin number) gets lost?

and i also don't have a card. yung number lang meron ako. but an employer needs to have the form or the card.

how will you get another copy? (if ever it is possible to get another one-- certified true copy)

Sir Tebs, I think the more important document is the TIN card if you were already issued one pero kung hindi pa, baka kelangan mo ng Affidavit of Loss para makakuha ka ng certified true copy ng form na nawala.
