help: lost TIN# form

what to do when your bir form 1902 (the 1st form you fill up when you get your tin number) gets lost?

and i also don't have a card. yung number lang meron ako. but an employer needs to have the form or the card.

how will you get another copy? (if ever it is possible to get another one-- certified true copy)
