lost driver's license

nawala lisensha ko, puwede ba ako kumuha ng panibago sa LTO branch na pinagkuhanan ko nito or kelangan sa head office pa (layo kase samen head office). ano ang requirements? (bukod sa affidavit of loss, para bang kukuha din ako ng bago na me mga medical, drugtests at kung ano ano pang mapagkakaperahan?) makakatulong ba kung meron akong xerox copy nung or/cr at card mismo? mgkano kaya damage? tnx!

nga pala 1 yr old pa lang yung lisensha so nde pa sa renewable i assume?

Regarding your q's, aside from an affidavit of loss, you need the bring a residence cert. & photocopy of your lost lic./or. My case, I wasn't required to undergo the diff. tests mainly bec. it was just a requesition for a duplicate copy. Maybe, ganon na rin sa yo. But they'll take a new pic kaya maha-hassle ka coz of the steps you have to go through. Grabe pa naman dami tao sa LTO everyday. They'll initially issue a temp. license, indexcard-size. You'll get the card after 3 mos. =)

AFAIK makukuha mo rin agad yung kapalit na license card as long as kumpleto yung mga requirements mo at mabayaran mo yung fee.

la akong binigay na residence certificate, la din medical. la din akong dalang xerox ng old DL ko.

affidavit of Loss lng. fill-up ka ng form, picture then bayad.

it took 2 hours sa San Juan LTO. This happen Aug last year.

Actually, it was misplaced hindi nawala dahil sa pagpapalit-palit ng wallet.:hiya: Since I'm on the road practically everyday, I decided to get a duplicate asap. That was last May. Btw, sir, female po ako.

I think this shld be the SOP for lost lic. I almost forgot, kaya card yong inissue sa akin, sira(daw) yong machine na nagpi-print ng card.

hmm ok na yung saken i just submittd afidavit of loss wala nang ek ek and ro ng dating license and or... i had it re issued rom th same branch whre i got it... sorry po miss inoba.. kala ko lalake ka hehh
