with the engine not running, the gadget reads battery voltage, which goes lower than 12 volts as the battery gets weaker.
with the engine running, the gadget will read not the battery's output, but the alternator's output (of at least 13 something volts), thus giving you the mistaken impression that your battery is strong even as it is about to die if not yet dead.
but if the alternator is defective, then, with the engine running, the gadget will register a lower voltage...
