Interesting. Looks like possible design flaw in the AC60 transmission used in Forts and Prados? The first link mentioned that the forward clutch package might be subject to too high pressures, which is likely why it's grinding the clutches/friction material when left in neutral.
LC200 uses a completely different tranny - AB60F.

Those are two different failure modes I think. While the C1 clutch drum problem seems to be present in all AC60 A/Ts, it seems though that only the Fortuners are affected. The C1 drum failure is likely caused by the Fortuner's TCM programming. Maybe it's commanding too much pressure to be applied on the forward clutch packs, which in turn leads to early failure of the drum ridge where the retaining ring is held. It's ironic because a high ATF pressure means that the clutches have little chance to slip and should, in theory, last longer. But it does mean that the shift-shock will be more prominent.
When clutch packs wear out excessively or becomes burnt however, it simply means it is slipping and consequently overheating. If any, a low ATF pressure can cause that, not high pressure.
The TCM is typically programmed to modulate the solenoid valves to provide a balance between having a smooth shift and maintaining clutch pack life.

Still going strong-my 2006 Civic just crossed 289,000 km last month. Regular maintenance really does go a long way. Curious to see who’s cracked 300k+!

Mine is a Ford Everest 2009 Model. Already 16 years young, has 178,784 on the clock and still going strong but not as strong as before. Not bad. BTW, just reactivate my account from a very long lay off.

I currently have a car with around 220,000 km (about 137,000 miles) on the odometer, and it’s still running pretty well with regular maintenance. High mileage doesn’t always mean trouble if the car has been properly cared for!
I’m curious — what’s the highest mileage any of you have reached with your car, and how did you keep it running smoothly?
