BTT: When it involves health, life and death, cost really takes a back seat. CVT, mas matakot yung college buddy mo if the wifey suddenly refused to pay up for the air service!!! I know of a wealthy family where the mom skimped on the care of her son who was in need of treatment. Pinabalik ba naman sa Pilipinas after learning of how much the treatment and care in the US would cost them (and they could well afford it).

Actually families have arrangements for Medevac to include assistants, or second assistant secretaries on credit.
Only those who have not chartered a flight would need to pay upfront.
It is always cheaper to pay an annual premium and not use it
than be somebody who is not listed.

Usually hindi pinag-uusapan naman kung magkano or ang mahal naman.

Well, buti naman inuna nila yung pasyente bago kinuha ang pambayad. At pwede mag-negotiate. You can't be expected to have that much money on you while you're out on a vacation to somewhere remote. Dapat pwede na ang credit cards, since may mga wireless terminals na ngayon. It is very expensive though, but at least they can cater to the need if it arises.
