GL Theta II 2WD (base variant, the one without passenger airbag and ABS + EBD) = Center lap belt
GL R-eVGT 4WD = Center lap belt
GLS Theta II to Premium package = Roof mounted 3 pt. seat belt

There are five seatbelts. Not sure if the rear center seat has a lap belt only or 3point ones like on the outboard seating positions.
