It's hard to generalize all doctors as mayaman. Some call center agents make more than some doctors I know. And the duty hours are horrendous (at least 24 hours per shift). Yung iba tumanda na sa pagiging moonlighters.
As for the OR, if it's the hospital you're paying through dapat talaga may resibo iyan kasi business establishment iyan (no different from Servitek or Rapide).
If you're paying directly to the doc or his secretary minsan wala na nga. But you can ask for one and you should not be denied.
Kapag sa hospital kasi pinadaan ang PF ng doctor, mangungumisyon pa ang ospital (plus the EVAT and other things). In the end mas mataas talaga babayaran ng pasyente. Unlike if direct transaction, the doctor will only charge for his true PF without the add ons.
Yung relationship kasi sa doctor ay parang relationship din sa shadetree mechanic. It's based on a handshake with no express nor implied warranty. Hindi din naman ako humihingi ng OR sa pinsan ni afrasay kapag nagpapaayos ako ng sasakyan.

Ang bottomline is: doctors are not exempt from paying EVAT, so they should issue OR's. No ifs, no buts. They should follow the law. Parang blackmail kasi kung sasabihin nilang: "Kung mag-iisue ako ng OR sayo, then tataasan ko ang bill ko ng 12%..."
The OR serves as a paper trail in case na kailangan ng BIR na mag-audit ng kanilang finances. Pag hindi nag-issue ng OR, they don't want to state their true income, thus making them technically tax evaders.
sinabi moh! i hardly see my children now. when they were young, schedules were more predictable.
